Dental Enrollment
Provide excellent dental insurance benefits for your employees with
ARI’s comprehensive dental plan that includes Types 1, 2, 3, and 4
services and a $1,500 contract year maximum.
Employees are eligible at the beginning of the month following 90 days
of employment unless they do not elect coverage, in which case they
must wait until the next enrollment period beginning May, 2010. Your
premium deductions begin one month in advance of the coverage
effective date. Program deductible per individual family is $100 with
no limits.
- Type 1 (Preventive Services) benefits include 100% coverage for oral exams, cleanings, bitewing X-ray (2 per 12-month period), space maintainers, pain treatment, sealants and full mouth X-ray.
- Type 2 (Basic Services) benefits include 80% coverage for fillings, anesthesia, simple and surgical extractions, endodontics, oral surgery and periodontics with no benefit waiting periods.
- Type 3 (Major Services) benefits include 50% coverage for crowns, inlays, onlays, dentures and bridges with no benefit waiting periods.
- Type 4 (Orthodontia Services) benefits include 50% coverage for orthodontia services with a lifetime maximum of $1,000, no deductible, and no benefit waiting period.
Regular employee rates are: Employee only: $30.88; Employee plus one
family member: $59.52; and Employee and family: $106.70
